概要 心不 全を伴わ ない葉間胸水欝留 の2傍 といわゆ るvanishing tumorの4例 とを比較検討 し,本 症(後 者)間 おける葉間貯 留胸水は周囲 の肺実質,胸 壁,心 との間で,力 学的間平衡状態 間あ るこ と 間気付 いた.こ の事実 を根拠 として,本 症 の発生機序を病態生理的 間次 のごと く確立 した.心 不全 の 際,肺 葉 は うつ血(必 須条件)を 来た し,含 気性が芝 しく,か つ硬 化す る.そ の結果各葉間肋膜は弾 力性のあ る平板状 となる.心 拡大(必 須条件)間 よる圧が,右 上中葉 お よび中下葉 間の相接す る肋膜 間対 し,縦 隔側 より胸壁側へ と平 行的 間働 き,そ の結果,葉 間肋膜 が紡錘 状~類 円型 間たわみ,中 間 に腔 が出現 し,内 部が陰圧 となる.そ の際水分 間富む 周囲肺組織 より容 易間葉閥腔 間濾...
